Unified Asian Communities would like to welcome you to sign-up for our Holiday Community Baskets!

In this form we are also taking suggestions for food or supplies you would like to see in the Holiday Community Baskets. This form will be open throughout the end of November so that we can best assess what our Community needs are. We truly would like to personalize these baskets to ensure you have a great holiday season! The Holiday Community Baskets will be delivered in December.
